​Data Recovery Service Authorization

SERVICE AUTHORIZATION:
The client (hereafter “you” or “your”) authorizes Lake Mary Computers (hereafter “Lake Mary Computers”, “we” “our” or “us”), its employees, and agents, to conduct an evaluation of the media provided to us to determine the nature of the damage and provide an estimate of recovery cost and timing.  The client also authorizes Lake Mary Computers to take all reasonable effort and measures necessary to determine recoverability and to recover data from this media and or equipment.

SERVICE LIMITATION:
By agreeing to this service agreement, you are stating that you understand that the media/data/equipment you are making available to Lake Mary Computers is already damaged, that data recovery efforts can result in further damage and that we will not be held responsible for this, or any other type of damage caused to this media.

OWNERSHIP:
You are the legal owner, representative, or otherwise having legitimate claim and right to the property or act on the owner’s behalf and all data contained therein sent to Lake Mary Computers.  Any property left with Lake Mary Computers and unclaimed for 45 days, will be disposed of.  At which time, Lake Mary Computers shall have no liability to the client or any third party.

As indicated by my signature below, I affirm that I am the legal owner of the property left with Lake Mary Computers for the purpose of a Data Recovery Analysis and Data Recovery if applicable. I authorize Lake Mary Computers to use whatever technical means necessary at their disposal during the data recovery process. This includes the use of third-party Data Recovery Labs at their discretion. I further agree that if I do not wish to complete the data recovery service after the initial analysis has been completed, I may have the property returned to me and agree that I will be responsible for any shipping and handling charges that apply. I have read and agree to Lake Mary Computers Data Recovery Policy Terms and Conditions, and Privacy Policy made available to me online at www.lakemarycomputers.com/datarecovery and www.lakemarycomputers.com/privacy prior to signing this authorization.
